Charting

  1. Case note/progress note is part of SW daily duties. There are perhaps many reasons why case note is needed, but I want to just mention two reasons for the purpose of our discussion. (1) To track client progress, (2) to justify our work, and this involves payment/reimbursement from funders.
    A couple of issues that seem to stand out for me: (1) Often times we spend too much time for documentation because more or less documentation is a form of our measurement/justification for our work. As a result, we do not have enough time to actually help clients resolving their problems as we should. (2) We are sometimes concerned about legal issues; especially, in clinical settings. As a result, it limits our ability to reflect our client needs as well as our service to our clients.
